First-Week Matters - CITI Completion Report

Your subsequent assignments will not be graded unless you successfully complete and submit this document and your mini-biography.

Summary:
You will be completing your CITI training to acquire your completion report. See "Example" link to see what a completed report looks like. If you have already completed your CITI training in a recent semester and the report matches the modules in the "Example" link, then skip the training and submit that completion form instead. Otherwise, go through the following steps below:
  1. Go to the "Training" link above.
  2. Follow the steps for the "Initial Training Course".
  3. Select and complete the course titled "Group 2: Social and Behavioral Research for Investigators and Key Study Personnel".
  4. Download and print the completion report. This report should match the required modules in the "Example" link.
In order for your form to be accepted, the Learner name must match your name, the required modules must be from the Group 2 training set, the date completed must be from within the past twelve months, and you must receive a 100% for each score-based quiz in the training set. Send me a form that has your name that shows that you passed.
